nissan truck bogging

Hello, own a 4wd 91 nissan truck,in line 4cyd.(Ka24e) . Starting out in first gear rolls few feet and she bogs down, then engine catches and starts to wind up, as I progress through gears, truck will hesitate , runs great at highway speeds, however I am confused, have throttle body fuel injection system. Also engine idle seems off, engine will shut down for a moment at the same point on accelerator pedal, any thoughts folks.
